Insurance

Due to the specialized nature of our practice, we participate with a limited number of insurance companies.

MEDICARE:  We currently participate with Medicare.  A prescription is required from a physician, physician assistant or nurse practitioner.  If you have been referred by your dentist, oral surgeon or chiropractor, please contact your primary care physician for a prescription.

PRIVATE INSURANCE:  We do not participate with most private insurance.  Please contact our office for further information and self-pay fees.  A prescription is no longer required for physical therapy.
